Now that we have that established, we can proceed.
Check the appropriate box (or boxes) to define the elements you need. Be sure you indicate how many of each selected form element you want. After you select which elements you require you will click the submit button at which time you will be given the opportunity to set the labels or captions for each form element you have requested.
